The correct answer is based on the fundamental principle of first aid on the road: to avoid aggravating existing injuries (PAS conduct: protect, alert, help). Cervical Protection: After an impact, it is very likely that the motorcyclist has suffered damage to the spinal column, especially the cervical vertebrae (neck). The helmet acts as a splint keeping the head aligned. Risk of Spinal Cord Injury: Removing the helmet without the proper technique (which requires two people) can cause sudden movements in the neck. This could sever or damage the spinal cord, leading to irreversible paralysis (paraplegia or tetraplegia) or even death. Exception: The helmet should only be removed if it is vital for immediate survival, specifically if the victim is not breathing (to perform CPR) or is vomiting and at risk of choking, and always attempting to keep the head-neck-torso axis aligned. The options regarding cold or comfort are secondary and trivial compared to the risk of severe neurological injury.
